Patient Survey

We are currently undertaking a patient survey at the surgery. Your perspective, as a patient, is crucial. Patient engagement is essential if we are to develop a successful working partnership for the future.

Out of Hours
During the daytime a receptionist can contact the doctor on call. After 6.30pm and at weekends, when the surgery is closed, your call will be automatically forwarded to the Badger Out of Hours Service. If you want to ring them direct, their number is

Home Visits
The doctor will visit any patient who is too ill to attend the surgery – please telephone before 9.30am wherever possible if you require a home visit.

Cancelling your Appointment
If you are unable to attend an appointment with one of the doctors or nurses, please telephone us as soon as possible, so the appointment can be used by another patient.

New Patient Registration

docandcomputerIf you wish to register at this surgery, please apply to the reception desk for further information.  If you change your name or move house, please remember to advise the receptionist.  Also remember you may need to inform the hospital if you are waiting for a hospital appointment.

The doctors welcome new patients who live within our practice area.  The practice catchment area (the districts from which our patients are drawn) is given on a map displayed in the foyer and includes Burton town centre.  If you move outside this area, you may be required to register with a new doctor.  The rights and responsibilities of patiens are included in our partien charter, which is displayed in the foyer, on this web site or a copy of which may be obtained from the reception desk.  We do not toleratoe violence or abuse and any patient using such behaviour will be removed from our list, and if appriate reported to the authorities. 

As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor.
